Application Checklist

Hourly/Salaried Applicant

• Copy of Social Security Card and Driver’s License
• Two consecutive pay stubs dated within 30 days of application
• Last two statements for all asset accounts (checking, savings, certificates of deposit, 401k,
  money market, retirement plans)
• W-2 Forms for last two years

Self Employed Applicants

• Copy of Social Security Card and Driver’s License
• Last two statements for all asset accounts (checking, savings, certificate of deposit, 401k, money market, retirement plans)
• Federal Tax Returns (including all schedules) from the past two years

For Purchase Transactions

• Copy of real estate contract

For Refinance Transactions

• Copy of current title insurance policy or abstract
• Last mortgage statement
